Medical Marijuana Certification for Minors

At CannabisMD TeleMed we believe that patients under the age of 18 deserve access to the natural treatment of medical marijuana. When dealing with minors we require a parent or legal guardian to be present. The medical intake forms that can be completed online also need to be signed by the parent or legal guardian. If the practitioner determines medical marijuana is an appropriate treatment then both the minor patient and the parent/legal guardian will be issued a medical cannabis certificate.

In order for a minor to be evaluated for medical cannabis we require the following:

• Medical records with the minor patient’s diagnosed medical condition (these can be faxed or emailed)

• Completed intake forms signed by the parent or legal guardian (completed through our online portal)

• Both the parent or legal guardian and the minor patient must be present for the virtual appointment

Medical cannabis certification evaluations for minors take longer than normal appointments. It is not common but the telemedicine appointment could last as long as one hour depending on the complexity of the medical condition. So please set aside adequate time for the medical marijuana evaluation.

We also provide unlimited follow-up appointments so the marijuana doctor can assess how medical marijuana is working for the patient. These follow-up appointments are provided at no additional cost to the patient and the frequency is determined by the physician.
